The Oregon Health Authority is requesting proposals from qualified individuals to act as consultants to facilitate the exchange of information between Coordinated Care Organizations (CCOs) and OHA.
Transformation Consultants will assist and support the CCOs in developing and implementing their transformation plans, which are focused on the integration of coordinated care for Oregon Health Plan members. Consultants will work closely with CCOs and be based in the community served by a CCO, ideally housed in the CCO office. The RFP will remain open until January 31, 2013.
There is one project Health Services Consultant 3 opening within the Division of Prevention and Community Health, Partnership, Planning, Policy and Operations Section – Health Communities Washington Project (HCWP).
The HCWP goals include: increasing tobacco-free living options; increasing access to healthy foods; increasing physical activity through safe routes to school and complete streets; increasing technical assistance for clinical providers; and integration of clinical services and community-based prevention resources. This project and position are anticipated to be funded through September 29, 2016.
