ASTHO Releases Request for Proposal for Creating Community-Clinical Linkages to Reduce Disparities in Hypertension ASTHO, with CDC’s support, is offering a request for proposal to help states achieve the Million Hearts goal of preventing one million heart attacks and strokes by 2017 through hypertension identification and control. The 2015-2016 ASTHO Million Hearts State Learning Collaborative will focus on creating, refining, and enhancing collaboration between clinical, public health, and community partners to create “systems of care” across clinical, community, and public health settings that identify individuals with hypertension and help them control their blood pressure.
